Referrals
If you are a doctor and would like to refer a patient to our facility, please fax our referral form with the patient’s medical records to the New Patient Coordinator at (405) 752-9650 or email. Once we receive this information, we will call the patient to schedule an appointment. We will then contact your office to inform you of the patient’s appointment date and time.
Information to include:
- Patient demographics
- Referral Letter
- Current Diagnostics (X-ray, MRI, CT, and lab work)
- Insurance information (front and back copy of insurance card)
- Medication list
- At least 3 office notes
Methods of Payment:
We accept most, but not all, major insurances and offer competitive private pay rates.
